Health Insurance Plan Options

Plan 13, 9, and 10's Pharmacy Program
Description of Plan 13, Plan 9, and Plan 13's Pharmacy Program (Pharmacy Program number and Health Plan number are not the same).

Plan 13 (Basic)
Description of United Healthcare's Basic Plan. With the Basic Plan, you pay a lower employee premium each month and pay more when using medical services.

Plan 9 (Basic Plus)
Description of United Healthcare's Basic Plus Plan. With the Basic Plus Plan, you pay a greater monthly employee contribution and lower out-of-pocket expenses when using medical services.

Plan 10 (Basic Choice)
Description of United Healthcare's Basic Choice Plan. With the Basic Choice Plan, you pay the highest monthly employee contribution, and the lowest out-of-pocket expenses when using medical services.

Enrollment

United Healthcare Enrollment Form
When enrolling for United Healthcare, print this form, fill it out, and bring it to Human Resources. Also, use this form if you need to: drop or add dependent(s); make an address change, name change, primary care physician change; or declare additions/change in status.

Health Insurance Waiver Form
Employees who have other health care coverage may opt out of the University sponsored Health Plan. Whenreturning the signed waiver form to your Human Resources office, you must include a copy of your health insurance I.D. card.

Health Insurance: Change in Family Status Form If you have had a change in family status within the last 30 days you may be allowed to make changes in your insurance coverage. Print and fill out this form and return it to Human Resources with the required supporting documentation and the United Enrollment/Change form.
