Health and Dental Insurance:
Your health and dental insurance coverage
will terminate on the last day of your employment. If
you choose to continue your health and/or dental insurance
coverage through the university, you will have 60 days
to make your COBRA election. ADP COBRA Services
is Tulane University’s selected COBRA administrator. You
will be receiving a package of information on your
COBRA rights and information on making a COBRA election. To

Life Insurance:
Your group life coverage will terminate on
the last day of your employment. You can convert
your voluntary coverage to an individual policy. Premiums
may change at this time. After your group coverage
ends, you will have 31 days to apply for the conversion
policy and pay the first premium payment. To
find out more about converting your coverage to permanent
life insurance, contact hr@tulane.edu. Members
of the Tulane University Medical Group with coverage
under The Standard should also contact The Standard
at 1.800.628.8600.
Retirement Plan:
Participation in the retirement plans will
terminate on the last day of your employment. To
remove or roll over money from the plans, you must
contact your retirement plan carrier directly at:

Flexible Spending Account Plans (FSA):
Participation in the FSA plans will terminate
on the last day of your employment. If you are eligible
to continue your Health FSA under COBRA, you must pay
for COBRA on an after-tax basis. To find out
more about your Health FSA COBRA rights, contact hr@tulane.edu.
You will have 30 days following the last day of your employment to file claims that were incurred on or before the last day of your employment. After that, any unused dollars are forfeited.
As you may have already heard, administration of the Flexible Spending Account plans will change from CONEXIS to Benefit Concepts, Inc. effective Jan. 1, 2006. Along with this change in administrators, we are eliminating the debit card option in 2006 due to the claim substantiation and service issues experienced by many employees last year.
Any claims submitted in December
should be sent to CONEXIS who provides three methods
of claims submission: E-mail
claims through the Participant website at My
CONEXIS. You must have access to a scanner to use
this method. Fax claims toll-free at 1.888.866.3312. Mail

Long Term Disability (LTD):
Your LTD coverage will terminate on the last
date of your employment. You will have 12 months
to make a claim for disability benefits for a disabling
illness or injury that occurred on or before the last
day of your employment. Members of the Tulane
University Medical Group should contact Unum Provident
at 1.800.858.6843, Monday through Friday, 7 a.m. to
7 p.m. central time for claim information. All other
faculty members with disability coverage should contact
CIGNA at 1.800.362.4462, Monday through Friday, 7 a.m.
to 8 p.m. central time.
Separation Notice:
A copy of your separation notification is sent to the
State of Louisiana Unemployment Security Office. This
notification will be used if you seek application
for unemployment benefits as a result of your separation
or resignation. Visit www.laworks.net for
more information regarding unemployment benefits.
Payroll/Personnel
Information Form:
Since you are receiving your final paycheck from Tulane
University, you are reminded that you will have taxable
earnings in this calendar year and, as such, will
be receiving a federal W-2 form in January of next
year that will be used for annual income tax preparation
and submission.
The Payroll/Personnel Information Form is online. If you have relocated, or plan on relocating or moving between now and January 2006, when your calendar year 2006 earnings statement will be mailed, please complete the form and submit it immediately to the Payroll Office. Following relocation, it is imperative that you complete this form to provide your correct address since the U.S. Post Office will not forward W-2s. Our inability to mail this documentation to you may cause major delays in timely receipt of your W-2. You will be assessed a $20 charge should you request a reprint of your W-2 and have not notified the Payroll Office of your correct mailing address.

Tuition Waiver:
The university will
honor the tuition waiver for the completion of your
dependent’s under graduate
degree as long as he/she has been accepted and is continuously
enrolled at the university. You will be required
to provide Human Resources with proof of her dependent
status on an annual basis, in accordance with the tuition
waiver benefit policy.
