DEVICE RELEASE


CLIENT  CONSENT FORM


     I have been provided with a device known as ____________________
("the device").  I understand that the device was developed and tested
as a student engineering project.  I understand that, although the
device has been approved by my therapist and by the designing student's
advisor, the device may contain defects in its design and/or
construction.  I have also been provided with an operation and safety
manual ("the manual") for the device.  I agree to operate the device
only in accordance with the manual.  I understand that the manual may
not provide full and adequate warning against defects in the design
and/or operation of the device.

     I hereby release, hold harmless and indemnify Children's Hospital,
The Administrators of the Tulane Educational Fund, Tulane University,
their employees, agents and students, and any and all persons involved
in the design and/or construction of the device and/or in the
production of the manual, from and against any and all liability, loss,
claim or expense (including attorney fees), whether based on tort,
contract, and/or products liability theory, that may arise out of my
use of the device.

     I agree to return the device to _________________________________
if and when it fails to operate normally or otherwise becomes unsafe
and/or unusable.
