BI-WEEKLY TIMESHEETS THANKSGIVING HOLIDAY SCHEDULE
notice of new mandatory social security reporting
A new federal law has been enacted that requires Tulane to report the Social Security Number for each of your covered dependents to our group health plan with United Health Care. Providing your dependent’s Social Security Number is not voluntary – it is mandatory. Please print the Social Security Number Reporting Form, complete the form and fax it to or mail it to WFMO. The form submission deadline is Monday, November 30, 2009. For more information about this requirement, visit http://www.cms.hhs.gov/Mandatoryinsrep/ or call WFMO.
