Supplementary Term Life

Tulane University provides you with Basic Life insurance coverage in the amount of 1.5 times your base annual earnings up to $50,000 at no cost. You may choose to pay for supplemental Term Life insurance for yourself or your family. Details and rates are in the Life Insurance Plan Basics document.

Statement of Health (.doc) ... The Statement of Health should only be completed if the employee elects coverage that is more than 2.5 times their annual salary.

Beneficiary Designation Form (.doc)
