FORMER EMPLOYEE INFORMATION

Resignation Responsibilities
- Submit resignation notice, turn in university ID
- Your department staff prepares all the rest: PAF, etc.
Last Paycheck
- Regular pay
- Vacation payout (if there is a balance of time for those accruing vacation time)
Health Insurance
- Ends on last day of employment unless you enroll in COBRA coverage
- Benefits will set up an account with the COBRA Administrator
- Respond to letter you will receive regarding COBRA coverage and HIPAA statement
- Your letter will include your account number, which is required to contact the COBRA Administrator in making your election. Log on to the COBRA Administrator's web site to make a payment.
MetLife Dental Insurance
- Ends on last day of employment unless you enroll in COBRA coverage
- Respond to letter you will receive regarding COBRA coverage
Retirement Accounts
- Moving your account (should be a link here to Direct Rollover Option in retirement plan)
- Withdrawals
- 403b (link here to Application for Benefits in retirement plan)
- 457b (link here to Deferred Compensation Plan )
- Keeping existing account, may keep current account except for those required by law to take a distribution
Life Insurance
- Ends on last day of employment or you may be eligible to convert to self-paid policy, contact Benefit Specialist, Linda Lambert.
Retirees
Same as for former employees, plus:
Addition under Health Insurance
- Special COBRA extension of up to 36 months if over 15 years of service and 62 years of age
