If an employee has been out for more than 3 days sick, do we have to offer Family Medical Leave?

Yes, if an employee has been out for 3 consecutive days and has not returned on the 4th day, Family Medical Leave forms should be sent from the Records Section of the Workforce Management Organization to the employee to inform them of their rights and obligations under the Family and Medical Leave Act.

 

I have an employee out on Intermittent Family Medical Leave how do I record their time?

The time is recorded in the sick/vacation column of the time sheet, indicating FMLA to the side of the hours.  A copy of the time sheet must be submitted to WFMO so that the time can be recorded and the hours tracked to ensure the employee does not exceed the 12 weeks.

 

Can you tell me the flow process for processing a Payroll Action Form?

If the Payroll Action Form is for a New Hire, Rehire, Salary Increase or Promotion, and is generated from the Health Science Center, it is routed as follows:

  • Department Chair
  • Office or Research or Financial Services (determined by account number)
  • Deans Office - Medicine - Dr. L. Lee Hamm
  • Deans Office  - SPH TM – Dr. Pierre Buekens
  • President’s Office – Anne Banos
  • Workforce Management Organization (Human Resources)
  • Workforce Management Organization (Payroll).

If the Payroll Action Form is generated from the Uptown Campus and is for New Hire, Rehire, Salary Increase or Promotion, it is routed as follows:

  • Department Chair
  • Office of Research or Budget Office (determined by account number)
  • President’s Office – Anne Banos
  • Workforce Management Organization (Human Resources)
  • Workforce Management Organization (Payroll)

If the Payroll Action Form is for Termination, Family and Medical Leave, Leave of Absence or Distribution Change, it is sent straight to President’s Office.

 

I have an employee that is terminating, who calculates the unused vacation hours due?

At the time you submit the Payroll Action Form to WFMO (HR), you must submit a copy of the final time sheet  Human Resources will calculate the hours due the employee before the PAF is submitted to Payroll.

 

How much vacation time is a terminating employee paid for?

The hours are determined as follows:

  • Employee working 37.50 hours per week and employed less than 10 years will be paid a maximum of 225 hours at the time of termination.
  • Employee working 40 hours per week and employed less than 10 years will be paid a maximum of 240 hours at the time of termination.
  • Employee working 37.50 hours per week and employed 10 years or more will be paid a maximum of 300 hours at the time of termination.
  • Employee working 40 hours per week and employed 10 years or more will be paid a maximum of 320 hours at the time of termination.
